I heard that “Travelers Liberty” plan is quite cheap and good for short trip, but don’t know about the reputation and claim settlement efficiency of the company who is offering this plan.

I don’t think it is mandatory unless they are traveling with a group that requires it.
Watch out…there are a lot of ripoffs out there. If it isn’t mandatory, don’t buy it. It isn’t a good investment.

No, they are not mandatory. However, remember that health care in the US is very expensive, so some health insurance may be a good idea.

Not exactally mandatory however, I believe that there is a requirement that the visitor have enough resources to be financially responsible.
